Just Heal & Smile: Smile Makeover Giveaway

1. Sponsor

The Just Heal & Smile: Smile Makeover Giveaway (“Contest”) is sponsored by Perfect Smile (“Sponsor”), in collaboration with Just Heal with Dr. Jay for promotional and educational purposes.

This Contest is not sponsored, endorsed, administered by, or associated with Instagram, TikTok, Meta, or any social media platform.

 

2. Eligibility

The Contest is open only to individuals who meet all of the following criteria:

  • Must be 18 years of age or older at the time of entry
  • Must be a legal resident of the United States
  • Must reside in, or be willing and able to travel to, Texas or Florida for treatment
  • Must follow @PerfectSmile and @JustHealwithDrJay on Instagram at the time of entry and at the time of winner verification (follows will be checked)
  • Must not be an employee, contractor, or immediate family member of Perfect Smile or Just Heal with Dr. Jay

Void where prohibited by law.

 

3. Contest Period

The Contest begins on February 1, 2026 and ends on February 28, 2026 at 11:59 PM Central Time (“Contest Period”). Entries submitted outside the Contest Period will not be eligible.

 

4. How to Enter

No purchase necessary to enter or win.

To enter, participants must:

  1. Follow @PerfectSmile and @JustHealwithDrJay on Instagram
  2. Complete the official Contest entry form during the Contest Period
  3. Upload a video submission (under 2-minutes) answering the prompt:

    “Why transforming my smile would change my life.”

Entries must be submitted through the official form only.
Written submissions, comments, direct messages, or emails will not be accepted.

Limit one (1) entry per person.

 

5. Winner Selection

One (1) winner will be selected based on the overall quality and authenticity of their video submission, including emotional impact, clarity, and alignment with the campaign’s self-love and transformation theme.

Winner selection will be conducted by the Sponsor in its sole discretion.

The selected winner will be notified via:

  • Email (using the email provided in the entry form), and
  • Direct Message on Instagram

If the winner does not respond within 48 hours, the Sponsor reserves the right to select an alternate winner.

 

6. Prize Description

The winner will receive a Smile Makeover Package, which may include:

  • Clear aligner treatment
  • Dental monitoring as part of treatment oversight
  • Initial consultation and treatment planning with a participating dental provider

Approximate Retail Value (ARV): Varies depending on individual treatment needs.

 

7. Medical, Dental & Treatment Conditions

  • All treatment is subject to a professional dental evaluation by a licensed dental provider
  • The winner must be clinically eligible for clear aligner treatment
  • Results are not guaranteed and may vary by individual
  • Treatment timelines may vary and may extend beyond the Contest Period
Scope of Treatment Disclosure

If the treating dental provider determines that additional dental work (including but not limited to extractions, restorative care, periodontal treatment, or other procedures) is required outside the scope of clear aligner treatment, such services:

  • Are not included in the prize
  • Are not the responsibility of Perfect Smile or Just Heal with Dr. Jay
  • Must be completed at the winner’s expense, if required, prior to beginning aligner treatment

All such matters are strictly between the winner and the treating dental provider.

 

8. Content & Publicity Release

By entering, participants grant Perfect Smile and Just Heal with Dr. Jay the right to use their submitted video, name, likeness, voice, and transformation story for marketing, promotional, and content purposes, without additional compensation, unless prohibited by law.

The winner agrees to participate in reasonable promotional activities documenting their smile transformation journey.

 

9. Taxes

The winner is solely responsible for any applicable federal, state, or local taxes associated with receipt of the prize.

 

10. General Conditions

Sponsor reserves the right to:

  • Verify eligibility and social media follow requirements
  • Disqualify entries that are incomplete, inaccurate, or violate these rules
  • Modify, suspend, or cancel the Contest if circumstances compromise its integrity
  • Interpret and enforce these Official Rules in its sole discretion

 

11. Limitation of Liability

By participating, entrants release and hold harmless Perfect Smile, Just Heal with Dr. Jay, participating dental providers, and their respective affiliates from any claims arising from participation, prize acceptance, or dental treatment outcomes.

 

12. Privacy

Information collected through the Contest entry form will be used solely for administering the Contest and related promotional activities and will not be sold or shared except as required by law.

 

13. Governing Law

This Contest is governed by the laws of the State of Texas, without regard to conflict of law principles.

 

14. Acknowledgment

By submitting an entry, participants acknowledge that they have read, understood, and agreed to be bound by these Official Rules.
